This thread's code appears to be a repurposed copy of the
City of the Dead code I posted in the Footage of the Unseen Level from Lost/Harbor City thread with a different level ID. That code was created long ago (wasn't derived from any of that thread's prior posts nor its associated YouTube videos/comments). Since it just forces CotD's level ID to always load, it can be easily adjusted to forcefully load any other level IDs.
As far as Tokyo Death Match itself goes, as Mosh mentioned, it isn't a scrapped level. Apart from CotD, there are no other scrapped levels nor vehicles present in the retail version of TMHOETE. I speak as someone who's researched it thoroughly in the past.

A clip of Tokyo Street's mini game can be found at the
42:36 minute mark in part 1 of Spazbo4's TMHOETE longplay video. It starts off with the following intro message:

Destroy Sweet Tooth heads to unlock stars. Collect all 7 stars in under 3 minutes to collect a bonus award. Finish under 4 minutes to keep powerups.
Based on those, unlocking Tokyo Death Match should just be a matter of collecting all 7 stars in Tokyo Streets' mini game within 3 minutes.

I don't own the PSP version of HeadOn though so someone else will have to confirm if Tokyo-DM is on it.

FWIW, I can also vaguely recall having played in that level in the online portion of the PSP version of TMHO ~12-13 years ago (as a standalone level).

... because Transylvania-DeathMatch, Jaffe's map exclusive to ps2, goes against original game design by not using that generic-DM song and going with Transylvania's music, so if Tokyo-DM were something he added to ETE too it probably would have used Tokyo's song.
Fun fact: The PSP version of TMHO contains Transylvania's background music and references to non-existent level files for it. So the TMHOETE version of that level might not have been designed from scratch. It was likely an enhanced/retooled version of the cut TMHO level. More information can be found in the last part of
