Didn't return for the same reason why Yellowjacket didn't return. The driver of both was Sweet Tooth's dad, and whomever you were in TM2 when you fought him, he died. Then in TM:B, his corpse was remote controlled by the young Kane(forgot his name, but cousin/sibling of Needles I think).

Simple as that. It is non-canon(as if a lot of drivers are).

If the team felt it necessary to reuse him, they wouldn't be doing their job to be creative designers & it would come off as a callow effort.

I think they over did it with Tower Tooth. Dark Tooth of Head On was an bigger black ice cream truck. Therefore it wasn't much different however the original Dark Tooth man. His vehicle was too big for you to play as at the time. Plus he had an alternate mode where his head goes flying around. Which would be very difficult to recreate.
